Assessments

Questionnaires clients fill in from their portal — how the staff view works today, and what clients see when one is assigned.

Assessments are structured questionnaires — intake forms, lifestyle audits, follow‑up check‑ins — that a client answers from their portal. Answers come back attached to the client, so you get consistent, comparable data instead of notes scattered through a chat.

Open Assessments at /assessments. (The Pending assessments card on your dashboard links straight here.)

The staff view

Two tabs:

  • Templates — every questionnaire your practice has, as a card showing its title, type, how many questions it holds, and whether it's Active or Inactive.
  • Assigned — who's been given what, when it was assigned, and when they submitted.

This is where you track completion — who's been sent a form and who hasn't answered it yet.

Current limitation — authoring and assigning

Creating a template and assigning it to a client aren't available in the staff interface yet. Both pages are read‑only: they show you what exists and where it stands, but there's no New template button and no Assign control.

So today the split is:

Templates and assignments have to be set up for you on the back end. If assessments are part of how you work, flag it through Get help at the bottom of the sidebar.

What clients see

The client half of this loop is complete. In their portal under Assessments, a client sees what's been assigned to them and taps Start to open it.

Questions can be any of: text, number, single select, multi‑select, scale, or yes/no. They work through the form and press Submit Assessment.

The moment they submit, it appears under your Assigned tab with a submission date, and the Pending assessments count on your dashboard drops by one.

  • Client onboarding — the medical tab is where structured clinical data lives.
  • Medical reports — the other route to structured client data, from uploaded documents.
